Consular personnel from the U.S. Embassy in Lima will visit Arequipa on Thursday, October 3, 2024. U.S. citizens in Arequipa may request appointments for the following services: voting assistance, passport renewals, registering birth abroad to U.S. citizen parent(s), and notarial services. A limited number of appointments will be offered to individuals who submit complete requests by e-mail as outlined below.
The time and location in Arequipa will be provided once the appointments are confirmed.
Services Available:
Voting Assistance
U.S. citizens wishing to vote in the 2024 U.S. general election can obtain information and assistance at FVAP.gov through the Federal Voting Assistance Program. If you need assistance navigating the process of registering to vote, requesting your absentee ballot, or mailing your absentee ballot, consular personnel can help. Send an email with the subject “Voting AREQUIPA” and your name to CuscoACS@state.gov.
Passport Application
Passport appointments will be available for adults and minors. As appointments are limited, adults eligible for mail-in application services are encouraged to use the mail-in passport renewal program.
To request an appointment, send the following documents to CuscoACS@state.gov with the subject “Passport Application AREQUIPA” and your name:
- DS-11 or DS-82 Form, filled out electronically
- Scanned copy of current U.S. Passport
Consular Report of Birth Abroad (CRBA)
CRBA appointments will be available to document U.S. citizenship of a child born abroad to a U.S. citizen parent or parents.
To request a CRBA appointment, follow these steps:
- Create a MyTravelGov. Watch this video for assistance.
- Access eCRBA and submit your application online.
- Submit payment ($100) on Pay.gov.
- Send us an email to CuscoACS@state.gov including the subject “eCRBA Appointment Request AREQUIPA” and the case number.
Once your e-mail with the eCRBA case number is received, the case will be reviewed and you will be contacted by email to schedule an appointment for an interview with the Consular Officer in Arequipa. The presence of the parents and the child/children is required on the day of the appointment.
