Consular personnel from the U.S. Embassy in Lima will visit Iquitos on Thursday, September 5, 2024.  U.S. citizens in Iquitos may request appointments for the following services: passport renewals, registering birth abroad to U.S. citizen parent(s), and notarial services.  A limited number of appointments will be offered to individuals who submit complete requests by e-mail as outlined below.

The time and location in Iquitos will be provided once the appointments are confirmed.

Services Available:

Passport Application

Passport appointments will be available for adults and minors.  As appointments are limited, adults eligible for mail-in application services are encouraged to use the mail-in passport renewal program.

To request an appointment, send the following documents to LimaACS@state.gov with the subject “Passport Application IQUITOS” and your name:

• DS-11 or DS-82 Form, filled out electronically
• Scanned copy of current U.S. Passport

Consular Report of Birth Abroad (CRBA)

CRBA appointments will be available to document U.S. citizenship of a child born abroad to a U.S. citizen parent or parents.
To request a CRBA appointment, follow these steps:

1. Create a MyTravelGov.  Watch this video for assistance.
2. Access eCRBA and submit your application online.
3. Submit payment ($100) on Pay.gov.
4. Send us an email to LimaCRBA@state.gov including the subject “eCRBA Appointment Request IQUITOS” and the case number.

Once your e-mail with the eCRBA case number is received, the case will be reviewed and you will be contacted by email to schedule an appointment for an interview with the Consular Officer in Iquitos.  The presence of the parents and the child/children is required on the day of the appointment.
