Consular personnel from the U.S. Embassy in Lima will visit Puno on Wednesday, May 8.
U.S. citizens may request appointments for the following services: passport renewals, registering birth abroad to U.S. citizen parent(s), and notarial services. Appointments are required and may be scheduled by providing complete information and/or documentation at the email address below.
Specific times and location in Puno will be determined when appointments are scheduled.
Services Available:
Passport Application: Passport appointments will be available for adults and minors. As appointment availability will be limited, adults eligible for mail-in application services are encouraged to use the mail-in passport renewal program.
To request an appointment, send the following documents to LimaACS@state.gov with the subject “Passport Application PUNO” and your name:
- DS-11 or DS-82 Form, filled out electronically
- Scanned copy of current U.S. Passport
Consular Report of Birth Abroad (CRBA): CRBA appointments will be available to document U.S. citizenship of a child born abroad to a U.S. citizen parent or parents.
To request a CRBA appointment, follow these steps:
- Create a MyTravelGov account. Watch this video for assistance.
- Access eCRBA and submit your application online.
- Submit payment ($100) on Pay.gov.
- Send us an email to LimaCRBA@state.gov including the subject “eCRBA Appointment Request PUNO” and the case number.
Once your e-mail with the eCRBA case number is received, the case will be reviewed, and you will be contacted by email to schedule an appointment for an interview with the Consular Officer in Puno. The presence of the parents and the child/children is required on the day of the appointment.
