Expired U.S. passports are no longer valid for return travel after June 30, 2022. Please read below for information on how to renew your current passport or to request a temporary emergency passport.

Applicants Age 15 or Under Without Both Parents Present
The DS-3053 is the Statement of Consent for Issuing a Passport to a Child and the DS-5525 is the Statement of Special Family Circumstances.
Adult Passport Renewal (DS-82)
Renew an existing passport issued to you when you were aged 16 or older.
Report and Replace Lost/Stolen Passport (DS-64 & DS-11)
Report a passport lost or stolen and replace it.
You can fill out the correct form, or find information about your passport status at https://travel.state.gov.
You must provide one photo on a white background with your passport application. Our photo policies apply to adults and children. We have a few exceptions to our photo policies for infants.
Emergency Passports
If you are a U.S. citizen with an urgent need to travel to the United States, please write to LimaACS@state.gov and include your current passport information, explanation, and evidence of your reason for travel, and a contact phone number. Emergency appointments will be considered on a case-by-case basis. Having purchased airline tickets without a valid passport does not constitute an emergency. Do not purchase a flight ticket until you have a valid passport.
As of February 27, 2022, the U.S. Embassy in Lima is able to receive certain adult passport renewal applications by mail. Currently, the mail-in renewal option is only available at certain DHL drop-off locations in Lima and Arequipa.
