The U.S. Embassy in Lima is pleased to announce that representatives from the Regional Federal Benefits Office will visit Lima to offer services for beneficiaries or individuals with questions about U.S. Social Security benefits.

Where: U.S Embassy, Lima – Avenida La Encalada cdra. 17 s/n, Santiago de Surco

When: July 18-22, from 8:00 a.m. to 4:00 p.m.

Appointments: You may schedule an appointment by contacting the FBU Costa Rica at FBU.COSTARICA@ssa.gov.  Please include in the Subject line “APPOINTMENT SSA – LIMA”. 

What to bring for Social Security services (please bring originals and legible copies of all documents):   

  • Applying for Social Security Benefits: Bring the originals and one legible copy of the following for all applicants: birth certificate and a valid passport.  If applying for auxiliary benefits or survivor’s benefits, please also bring a marriage certificate and/or death certificate.
  • Social Security Number Card Application: Bring the originals and one legible copy of your valid U.S. passport, Consular Report of Birth Abroad or original birth certificate, and completed SS-5FS form.
